David Rex Lewis, Jr, William Lewis and David Keith Lewis.
Five Daughters: Nettie Hendrix, Peggy Eversole, Martha Faye Lewis, Angela Lewis- Pennington and Misty Dawn Roberts. Three Brothers: Bill North, George North and John R. Collett, Jr Four Sisters: Betty, Evelyn, Claudine and Juanita. Also survived by 17 Grandchildren, 2 Great Grandchildren and a host of other 
relatives and friends. 
Preceded in death by her parents: John R. Collett and Nettie Whitehead-Collett 
and 1 son: Earnest Collett. 
Time of Service: Wednesday, Sept. 14, 2005 at the Dwayne Walker Funeral Home 
Chapel in Hyden,KY and Place of Interment will be at the Bowen's Creek Cemetery 
in Essie,KY.
