Seldon R. Sanders aged 50 years, a resident of Bee Spring, in the County of Edmonson and State of Kentucky who, being duly sworn according to law, state that “he is” acquainted with William W. Davis applicant for Invalid Pension; and know the said William W. Davis to be the identical person of that name who enlisted or volunteered as a Private in Company “E” 11th Regiment of KY Inf. Vols. and who was discharged at Louisville, KY on or about the 3rd day of July 1865 by reason “I suppose of expiration of term of service. I was discharge sometime before he was. That the said Wm. W. Davis while in the line of his duty, at or near in the State of did, on or about the day of 186 become disabled in the following manor, viz: I cant recollect just when and where I first heard him complain – I did not have the chance that others did but I recollect something about his having rheumatism while in the Army. I don’t recollect ? about his other disabilities claimed for I did not see him when he was discharged as I was discharged before he was. That the facts stated are personally known to the affiant by reason of I was not with the company all the time but I saw the claimant frequently and I heard him complain of rheumatism. I heard him complain at Bowling Green KY. I don’t know whether he was treated by Reg’t. Surgeon or not. And deponent further state that he was well acquainted with the claimant having known him for at least 30 years and further that my knowledge of the facts above stated is derived from said acquaintance, and from having served as Lieut” Of Company E of the 11th Regiment of KY Inf Volunteers, from the day of , 188, to the 16th day of Dec 1864. Davis complaining of Rheumatism & Neuralga also of piles in fact he had piles so bad he was exempt from duty several times. I heard him complaining off and on until I was discharged of Rheumatism & Neuralga. I was discharged in Dec 64 and he being a recruit was not discharged till some time in 1865. Edward Blair aged 61 years a citizen within the County and state aforesaid who I certify to be respectable and entitled to full faith and credit on oath he being duly sworn says that he has known the claimant for the last 40 years. I knew him when he enlisted we both enlisted in to the same co & Regt at about the same time at that time he was sound and free from neuralgia rheumatism or Piles. I recollect that while we was at or near Knoxville Tenn in the winger of 1863&4 he was sick he said that he had rheumatism and had his head tied up & said that he had neuralgia – sometime before that – and while we was near Sweetwater Tenn I heard him complain of his having Piles. I did not see Piles – and only know that I heard him complain of the same. He was bothered with rheumatism & neuralgia also complained of Piles – and he was disabled for duty at least on half – we were both discharged together and come home together he complained of these same diseases after he got home and while we were on the way home. I have lived a neighbor to him ever since his return home and have lived in from 1 ½ miles to two miles of him ever since. I have heard him complain quite frequently of neuralgia rheumatism & Piles ever since his discharge I can’t tell all the times I have heard him complain – and he has not been able to do over one third labor – at any time and for a considerable part of this time he has not been able to do anything at all. At date of his enlistment –also that they have lived neighbors to him ever since date of his discharge and return home and have lived within from 2 to 4 miles of the claimant most of the time since his discharge and have and do know his physical condition ever since his discharge and return home. Davis aged 41 years a resident of the (blank) of Brownsville county of Edmonson, State of Kentucky, who, being duly sworn according to law, declares that he is the identical William W. Davis who was ENROLLED on the 6th day of September 1862, in Company E” of the 11th Regiment of Ky Inft. Vols commanded by Capt Woodford M. Houchin and was honorably DISCHARGED at Louisville Ky on the 3rd day of July 1865; that his personal description is as follows: Age 41 years; height 5 feet 11 1/2 inches; complexion, light; hair, light; eyes Blue. That while a member of the organization aforesaid, in the service and in the line of his duty at Knoxville, in the State of Tennessee on or about the (blank) day of November 1863, he contracted Diarrhea and Camp fever which produced piles also on or about the same time he contracted Rheumatism and Neuralgia of which he still suffers and he claims a pension on account of Piles Rheumatism and Neuralgia which was contracted in Service. That he was treated in hospitals as follows: That he was not treated in any regular Hospital for said diseases but was treated in camp for said diseases. That he has not been employed in the military or naval service otherwise than as stated above. That since leaving the service this applicant has resided in the County of Edmonson in the State of Kentucky, and his occupation has been that of a Farmer. That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a Farmer. That he is now so much disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. Personally came before me, a County Clerk in and for aforesaid County and State, Seth H. Davis aged 72 years a citizen of the Town of Stockholm, County of Edmonson, State of Kentucky, well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case, as follows: I knew William W. Davis the claimant and enlisted into the same Company and Regt with him at the same time. I regarded him a sound and healthy man in every particular he remained with us seemingly well till some time in the winter of 1863 & 4 at or near Sweetwater Tenn. I heard him complaining of his leg which was caused by rheumatism. I heard him complain of Piles. Just before the siege of Knoxville Tenn this was late in the fall of the year 1863 he came in to camp from on scout suffering with Piles. I heard him complain a great deal with neuralgia. I can’t recollect all the times and places that I heard him complain of neuralgia in the army but I recollect that he complained a good many times of rheumatism, Piles and neuralgia. I was present with him and we were both discharge at the same time he was then complaining of these diseases we came home together from the army from the time we came home from the army up to this time we have lived in from ¼ to 5 miles of each other with exceptions of two months he has continued to complain of these diseases Piles Neuralgia and rheumatism – and he has been disabled as much as two thirds for the performance of manual labor ever since discharged. Just after we come out of the army he got down with rheumatism & Piles & lay for about 3 months, that he was not able to get up some time after that he like to have lost one of his eyes on acct of neuralgia which injured the nerves of his eye he further declares that he has no interest in said case and is not concerned in its prosecution. Garret S. Davis says that he saw claimant in about a week or ten days after he came home and when he saw claimant he was sick he looked like he was sick and said that he was sick he then told me that he had Piles, rheumatism and neuralgia – he was stiff in his knees and halted in his walk – Garret S. Davis says that he cant recollect just exactly of what claimant complained but does recollect that he was not well in some what that they both have lived in from a few hundred yards to five miles of him nearly all the time ever since he come home from the army and that claimant has been complaining more or less all of this time with Piles Neuralgia or rheumatism – that he got so bad off with neuralgia that he come very near loosing one of his eyes that they have frequently seen him with his head bandaged on account of neuralgia. Wm C. Guess says that he could not ride on horseback on acct of Piles – and showed him on his finger about how far his bowel protruded – they both say that he has complained of rheumatism frequently and Guess says that he has seen him with rheumatism so bad that he limped and halted in his walk and that he has been as much as one half disabled for manual labor – he has been complaining of rheumatism Piles and neuralgia and from what we have seen of him and from his many complaints – he has been disabled equally as bad as he claims they further declare that they have no interest in said case and are not concerned in its prosecution. Vols on this 12th day of June A.D. 1882, personally appeared before me, a County Court Clerk in and for the aforesaid County, duly authorized to administer oaths, William W. Davis, claimant, aged 41 years, a resident of Brownville in the County of Edmonson, and State of Kentucky, well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to his claim for pension as follows: My Post Office Address is Brownville, Edmonson County Kentucky. For five years immediately preceding my enlistment into the service of the United States on the 6th day of September, 1862, I resided in the following –named places: Edmonson County Kentucky and my P.O. Address was Brownville Kentucky, and my occupation was that of a farmer. Since my discharge from said service on the 3rd day of July 1865, I have resided in Edmonson County Kentucky and my P.O. address has been Brownville Kentucky and my occupation has been that of a farmer so far as I have been able. I further state that the disability for which I claim a pension arises from Piles, Neuralgia & Rheumatism which was contracted November 1863 at Knoxville, Tenn, I was taken sick with a severe attack of fever and Diarrhea caused from over heat and fatigue produced by marching &c, said fever settled in my system which produced Neuralgia and Rheumatism and Piles was produced by said “Diarrhea”. From my said discharge to the present time, I have received the following medical treatment for said disease: Dr. J.T. Baker of Rocky Hill Sta. Ky has treated me, Dr. Davenport , Dr Broadie of Rockie Hill Sta Ky and Dr H.G. Cohnes of Brownville Ky have all treated me more or less for said diseases but I do not recollect the exact dates at the present, besides I have used various kinds of remedies recommend by friends and neighbors but have derived little or no relief from either of Doctors or Remidies used. Since the origin of the disability for which pension is claimed, I have suffered with the following acute diseases: I have suffered but very little with any disease of a special or acute character. Sometimes I have slight-attacks of fever but it is very seldom for which I was treated by Dr. Davenport whose P.O. address I do not now know also by Dr. J.T. Baker of Rocky Hill, Ky. And during all of the said time my physical condition and ability to perform manual labor has been as follows: Sometimes I can do tolable well in the array of labor (farming for three or four months at a time). Then there will be sometimes as much as two or three months at a time I am not able to do anything and very often confined to my bed such has been the case ever since discharged. Upon examination we find the following objective conditions: Pulse rate, 72, respiration, 17; temperature: 48; height 5 feet 11 inches; weight 141 pounds; age, 51 years. Claimants general appearance is fair. Shows some ? suffers from 3 exterior piles high up ? tender and ulcerous at one point, veins of rectum engorged. (*very difficult to read!) Suffers stiffness of back and hips and complains of almost constant pain in lumbar region running down course of left hip muscles up left hip and and thigh soft and show some slight atrophy. We find all other joints to be in normal healthy condition. Heart normal in rate size and pulse and no abnormal pounds. Claims to suffer periodic neuralgia of head and eyes. The latter being a little red and watery, but vision is not diminished to any perceptible degree except the above no other disability found to exist. Had rheumatism near Atlanta Georgia in 1864 affecting shoulders and breast. Had neuralgia at same time which affects head and eyes. The examination reveals the following objective facts in support of his statement: Examination with speculum reveals no evidence of piles or ulsers. Tonge, skin, liver spleen and all of the abdominal viscera in normal healthy condition. Heart hypertrophied and quickened in its action. No evidence of valvular disease. Joints muscles tendons in healthy normal condition. Hypertrophy of heart in all probability due to past rheumatic troubles though there is no present evidence of rheumatism. No evidence of neuralgia. From the existing condition and the history of this claimant, as stated by himself, it is, in our judgment, (blank) probable that the disability was incurred in the service as he claims and that it has not been aggravated or prolonged by vicious habits. He is, in our opinion, entitled to a 4/18 rating for the disability caused by disease of heart –0- for that caused by Piles and –0- for that caused by Neuralgia and Rheumatism the sum of which aggregates 4/18. I am unable to tell how it was ? to be the 23rd on one of the affidavits except that it must have been a mistake in the draftsman for I know I gave it as the 29 – I have no means except my husband left me some little personal property appraised at $288.55 and a life interest in a rough tract of land of 50 acres of which only 12 acres is plowed and there is no other information on it I have no other income whatever. The exact value of the land I suppose is $12 to $15 per year. The personal property consists of 2 horses, 2 head of cattle six hogs household and kitchen furniture and some old farming implements. All appraised it is $288.55. The personal property is there on the place where I live and the land on Green River in Edmonson County Kentucky. I further state that the following affidavit was written by J. S. Lay of Brownville Ky in my presence at my house from an oral dictation and I was not aided assisted or in any way ? by any ? or written communication whatever in the preparation of the affidavit which was written Aug 14th 1897. I owned one cow part of the line since June 5, 1897, & have sold her about June 1st 1899 for $25.00 this is all the stock owned by me had to sell cow to support my children my intire household & kitchen furniture is not worth more than $25.00 my intire income for year since soldier’s death has been nothing only what I have worked for no surplus after feeding & clothing my children my income is less than $50.00 per year no Pension has been legally bound to provide for me or my support. I see that the date of the birth of Malissie is given as July the 18th 1889 and Lena B as Aug 24th 1892 and Nat S. as of Nov 7th 1896 which accords with my best recollection and while I do not know positively the date of their births I believe that the ages given by the Claimant in her affidavit are correct. Davis and Mary.. T. Merideth was married in July 1860 then Mary. T. Davis Died July 22 1878 this is what my Sister Sayes that the record in the famley Bible Shoes the Bible that has the records of the marridge of Soldier William W. Davis and the deth of his first wife mary T. Davis is in illenois and wee canot have it before us the woman that has got the Bible is the oldest daughter of William W. Davis her name is olley. Simmons aged 37 years Whose post office address is Pig Edmonson County Kentucky who being by me duly sworn states in reference to the aforesaid claim as follows I am well acquainted with the claimant Kizzie J. Davis and have been ever since I can recollect and was intimately acquainted with the decedent William W. Davis at the time of his death which occurred April the 3rd 1897 and have been for about 13 years. I have lived a near neighbor to the decedent and his wife the Claimant for about 13 years and I was called as one of the Lady attendants in fact was the only one at the birth of their child Parker D. Davis which according to my recollection accured June the 13th, 1894. I was the midwife on said occasion, I lived as before stated a near neighbor to the Claimant and the decedent William W. Davis and visited them frequently and was to see the decedent William W. Davis often in fact every day and some days twice a day for the two weeks preceeding his death. I was at his house only about three hours before his death on April the 3rd 1897 & also went back to the house of the decedent the next morning and saw him lying dead and he was buried on April the 5th 1897 and I attended the burial and saw him interred and can state positively that he is dead. I further know that the Claimant Kizzie J. Davis had been previously married to Harrison Isenburg who died as I remember it August the 7th 1878 I know he is dead I was at his house where he was sick a little while before he died I then went home and staid two or three hours and went back to Mr. Isenburg’s and he was dead. I was at the burial and saw him interred. As I before stated I live a near neighbor to the claimant and did when her husband William W. Davis died and I know she has not remarried since his death for I live so close and am at her house so often and she at mine she could not have remarried without me knowing it. I know that their children Jesse H. Davis Malissie A. Davis Lena B. Davis Parker D. Davis and Nat S. Davis are all living. I further state that I have no interest in the claim and am not concerned with the prosecution of it and that the foregoing statements were written by my oral dictations in my presence by J.S. Davis died I am the man who closed his eyes I also made the Coffin and saw him buried. I am the husband of Mary V. Simmons and my age is 53 and my postoffice address is Pig Edmonson County Kentucky. I now live and have for about 13 years a near neighbor to the Claimant and the decedent infact I live in sight of their house I was present at the marriage of the decedent William W. Davis to the Claimant which occurred at the residence of John S. Perkins the 17th of Dec 1885 they were married by Rev. Benjamin Sanders who is dead I live so close and have ever since the death of the decedent William W. Davis that if his widow the Claimant had remarried I would have known it I know that the Claimants children Jesse H Malissie A Lena B Parker D and Nat S Davis are all living. I know the full name of the youngest child is Nat Sherman Davis. I am well acquainted with the property that the Claimant because I live a near neighbor and was one of the Appraisers of it after the death of William W. Davis and it consists of the following two head of horses two head of Cattle some hays household and kitchen furniture and some inferior farming implements all appraised at $288.55. He owned fifty acres rough thin land with no ? Provinention it which the Claimant only has a life interest in it and there is only about 12 acres cleared the result if this land would not be worth more than $12 to $15. The Claimant has no other source of income whatever except the above letter personal property and the rest of the 12 acres of land which is not sufficient to feed cloth and educate her children. She (the claimant) has no one who is legally bound to support her. I further stat that the forgoing affidavit was written by J.S. Davis, who served as a private in company “E” of the 11th regiment of Kentucky Infantry Volunteers. On this 21st day of January, 1898, personally appeared before me a Clerk of the County Court within and for the county and state aforesaid duly authorized to administer oaths, J.B. Minyard aged 32 years a resident of near Pig in the county of Edmonson and state of Kentucky, whose post office address is Pig Ky. Well known to me to be reputable and entitled to credit, and who being duly sworn according to law declared in relation to aforesaid case as follows: That he as well and personally acquainted with the deceased soldier, William W. Davis for 20 years or more and have been living a near neighbor to him for almost 4 years. That he was with him during his last illness and was present when he died, and now states that the said William W. Davis died on the 3rd day of April 1897. Affiant says that the reason he is able to fix the date of is death exactly is that he knows that said Davis died on Saturday night before the first Monday in April, and the reason he fixes the first Monday is because it is County court day in Edmonson county. Affiant further states that he is acquainted with the property of the widow, Kizzie J. Davis, left her at the death of the said William W. Davis, as he was one of the appraisers who valued the property after the death of the said Davis, and to the best of his recollection the personally property was valued at about $287.55, and the real estate that is clear of lien or incumbrance is worth not exceeding $75.00 and for renting purpose is not worth more than $12.00 per year. That the remaining portion of said farm listed for taxation by the said W.W. Davis is now in dispute pending a judgment of the Edmonson Circuit court. Affiant further states that the claimant Kizzie J. Davis has no income of any kind or any other means of support other than her daily labor. Perkins, aged 34 years, a resident of near Rhoda, Kentucky, well known to me to be reputable and entitled to credit, and who being duly sworn according to law declares in relation to the aforesaid case as follows: That he was well and personally acquainted with the above mentioned William W. Davis, who was the husband of the claimant, Kizzie J. Davis, That he is also well and personally acquainted with the claimant herein and knows that she is the widow of the same deceased soldier William W. Davis. That he is a merchant, and as such, deals in Dry Goods, clothing, etc., and that he furnished the burial clothing for the said William W. Davis and carried them to him after he was dead and that he saw the said Davis after he was dead and before he was buried. That he entered the date of the furnishing of the aforesaid burial clothing etc, in his books kept for that purpose, which books show that said burial clothing was furnished on the 5th day of April 1897, and he is therefore able to state that the said William W. Davis died on the 3rd or 4th day of April 1897.
