Additional Comments:
1. The name on the certificate looks like "Bird Allice Flack". However, it's 
possible the middle name is Allen. It definitely begins with "All__".

2. As always, the signatures of the doctor, undertaker and, in this case, the 
informant are difficult to read. I might be wrong on any or all of them.

3. It looks like the city is "Glencoe", but I might be wrong.
