Name of spouse not given on the death certicate. However, I know her name was 
Lula Belle (Kelley) Flack because they are my grandparents. Additionally, the 
name of the informant was given as: Mrs Lula K. Flack. That's an error. It 
should have been: Mrs. Lula B. Flack.
