the "11" portion is certain.

5. The death certificate states that she stayed for 50 years in the Rest Harbor 
Home. However, maybe this is a mistake. It seems like a long time. Perhaps, 
they meant that she lived in Lockport, Henry County, Kentucky for 50 years.

6. Doctor's statement: "I hereby certify that I attended the deceased from Feb. 
1, 1949 to Feb. 18, 1949, that I last saw the deceased alive on Feb. 18, 1949 
and that death occurred at 11:30 A.M. from the causes and on the date stated 
above.

1. The father's name is misspelled. It should be James J. Flack. The mother's 
name is also misspelled. It should be Margaret Jane Chisholm.

2. The name of the husband is not stated. However, she was married to Sam 
Thomas, Commonwealth of Kentucky, Certificate of Death: Vol. 024, Certificate 
11721, Death Vol. 1949.
