Mrs. Sally Neill Roach and child a native and loyal citizen of the U.S. , do hereby apply to the Dept. of state at Washington for a passport for herself and one child born at Louisville,Kentucky on the 4th day of February 1851 and her son Ethrie (Elkrie)born in Louisville,Kentucky October 2nd 1877.
In support of the above application,that i do solemly swear that i was born at Louiville, Kentucky on or about the 4th day of February 1851,my permanent residence being at Louiville in the state of Kentucky, that i am about to go abroad temporarily and that i intend to return to the U.S. in about a year.
