Additional Comments:
1. The birthplace was incorrectly spelled "Seria". I believe it should have 
been spelled "Syria".

2. I spelled the name "Rest Harbor Sanatorium" exactly as it appears on the 
death certificate. The last word is probably misspelled.

3. The name of the spouse is not stated on the death certificate. However, the 
wife's maiden name was Icey Flack, daughter of James J. Flack and Margaret Jane 
Chisholm, both of Henry County, Kentucky.
