Commonwealth of Kentucky, Certificate of Death, Vol. 024, Certificate 11721, 
Death Vol. 1949 for Sam Thomas of Lockport, Henry County, Kentucky. Sam was the 
son of Sam Thomas (Senior) of Syria. He was married to Icey Flack Thomas, also 
of Henry County, Kentucky. The informant for the certificate was James Thomas. 
He was buried in Pleasureville, Kentucky. The registrar was Mary M. Mahoney and 
the funeral director was Ralph Willhite.
