The Commonwealth of Kentucky
Whereas, James Storm, has been appointed, by the County Court of Laurel County, and has qualified as Guardian to Wm. Storm, Margarett Storm, Zerelda Storm, Hardeman Storm, Nancy Storm and Polly Storm.

Now, we, James Storm as principal and William Storm his sureties, do hereby covenant to and with the Commonwealth of Kentucky, that the said William  James Storm will faithfully discharge the trust of Guardian to said minor, in all respects as required by law.
