We the inhabitants of Falmouth and its vicinity being members of the Presbyterian Church do by voluntary consent that we may enjoy the ordenances of Jesus Christ form ourselves into a church to be known by the name of the Presbyterian Church of Falmouth, Pendleton County, this 5th day of December 1829 and we do moreover choose to the office of Ruling Elders in the church James Naylor and Devall Payne, Jnr. and we do voluntarily agree to submit to them in the due exercise of their office.  Done by order of Presbytery.
