Personally came before me, a Clerk of Carter County Court and for aforesaid County and State, A J Scott aged 70 years citizen of the Town of Grayson, County of Carter, State of Kentucky, well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case, as follows:

My post office address is Grayson Carter County Ky. I was a member of Co. D 40th Reg. Ky. infatr vols. I am acquainted with Martin Lunsford and have known him ever since the year of 1857. Before claimant volunteered into the service of the United States he was a sound man in every respect and free from disease of the heart as far as I knew and that while in the line of duty at Paris, KY sometime in the summer of 1864, Claimant was sick and complaining of shortness of breath and palpatation of the heart. I heard him complain upon difent times while in the service of shortness of breath and palpataions of the heart and I have heard claimant complain of disease of the heart upon difent times since the war down to the present time, I know all the fergoing statements from my own personal knowledge. 

I further declare that I have no interest in said case, and am not concerned in its prosecution
