Kentucky GenWeb projectEulaliah P. Levack 1926 – 2007 MOREHEAD – Eulaliah Smith Levack, 80, of Mansfield, Ohio passed away Monday, April 16, 2007 in the Crestwood Care Center in Shelby, Ohio. She was born on September 14, 1926 in Soldier, Kentucky, a daughter of the late Frank and Pearl Conley Smith. She was the niece of the late Rev. Russell Smith of Haldeman. Ms. Levack was retired from General Motors and was a member of the Church of God in Mansfield. In addition to her parents, she was preceded in death by a daughter, Sandra Jarvis, two brothers, Harold Smith and Bill Smith and four sisters, Edna Smith, Elana Stamper, Dora Holbrook and Shirley Legg. Survivors include two sons, Randolph Underwood of Mansfield, Ohio and Larry Underwood of Kansas City, Kansas, six grandchildren, seven great-grandchildren, a brother, Clifford Smith, a niece, Chlotene Noble and sister-in-law, Inez Smith. Funeral services will be conducted at 10:30 a.m. Friday, April 20, 2007 at the Ontario Home of Wappner Funeral Directors in Mansfield with Pastor Rich Hurles officiating. Burial will follow in the Oak Grove Memorial Park. Contributions are suggested to the American Heart Association. Lane-Stucky-Gray Funeral Home provided this obituary information.
